## ★★ Pipeline Malfunction 観測 (新規 issue, high severity)

**3 layer 同時 bypass**:
- STEP 1087 `requireSourceDiversity` (within-batch source rotation, default ON) → invention-1 + invention-5 完全同一 source を leak
- STEP 1190 `requireFreshSourceVsHistory` (60-day source ban, default ON) → invention-1 「C1-C5 意識=情報統合 Φ>0」 vs approved-2026-05-30 transplantSource 完全一致を leak
- STEP 1212 `requireSourceVsHistoryStrict` (strict full literal ban, default ON) → 同上

**仮説 3 種**:
1. cron path の `InventionEngine` instantiation options が `DailyInventionReporter` 経由でない可能性 (manual scripts 経由で default OFF)
2. literal normalization edge case (`extractTransplantLiteralsFromHypothesis` で 「」 enclosure parse + `normalizeAxiomLiteralStrict` で stored vs extracted 不一致)
3. recent commit (06-19 以降) で hardening 機能の regression

**STEP 1228 (v) `Pipeline Malfunction Diagnostic` — 2026-06-22 完遂 (commit `253cfe9e3`)** ✅:
- ★ **真の root cause = kernel pool truncation in cron path** (recent commit regression ではなかった)
- `scripts/daily-invention-entry.ts` line 14-23 が `src/axiom-os/seed-kernel.ts` 1 file のみを単行 regex で scan → **271 theories のみ** (true SEED_KERNEL 1650 の 16.4%)
- 副作用: optional fields `addedDate` / `cooldownDays` も regex で missing → **STEP 1088 cooldown 機能 cron path で完全無効化** (30 件 → 0 件)
- 06-20 per-category 実 pool: consciousness=3 / general=3 / mathematics=14 / logic=13 (小 pool で STEP 1087/1212 rotation 候補枯渇 → graceful fallback → leak)
- Hardening layers (STEP 1087/1190/1209/1210/1211/1212/1216/1225/1226/1227) は **正常動作していた**、 ただし「ALL candidates banned」 時の graceful fallback が小 pool で常時 hit していた
- **Fix**: regex extraction を削除 → `import { SEED_KERNEL } from '../src/axiom-os/seed-kernel'` 直接 import (seed-kernel.ts 内部で全 phase / civilization / approved-inventions を import 集約済)
- **Verified**: 271 → 1650 theories (+509%) / consciousness 3 → 5 / logic 13 → 15 / STEP 1088 cooldown 0 → 30 active
- ★ honest scope: pool 全体は 6x 拡張だが、 SEED_KERNEL 全体でも tiny な category (general=3, consciousness=5) は依然 collision risk あり。 完全解消は future STEP 1228 (z) `dropVoidOnSourceRotationExhaustion` (STEP 1227 (u) pattern を source 側にも適用) で対応 trigger

**(z) candidate (future, 本 STEP 範囲外)**: STEP 1227 (u) `dropVoidOnWithinBatchExhaustion` (within-batch target rotation 失敗 → drop) の source 側対称 mirror。 `requireSourceVsHistoryStrict` rotation loop が全 candidates ban で枯渇した場合、 graceful fallback で original 保持せずに void を batch から drop。 trade-off: 5 件 batch + leak vs 縮小 batch + leak 0。 06-22 以降の batch 観測で necessity 判断

## 新規 hardening candidate (本 audit から提案)

- **(w) `requireRetireStuckTargets`**: recycle count ≥ 10 件 (例: 「記録事実は不変」 16 件) で transplantTarget pool から auto-retirement (365-day cooldown or permanent removal)
- **(x) `requireSirenClaimDetector`**: literal 内に 「世界初/独自/唯一/world-first/unique to/proprietary to/D-FUMT独自/Rei独自」 等 uniqueness keyword 出現で audit gate (auto-hold + 人手 confirm 必須)
- **(y) `requireWithinBatchCrossDirectionBan`**: STEP 1211 (m) within-batch target uniqueness の拡張、 batch 内で target → source cross-direction chain (例: invention-2 target = invention-3 source) も ban

## Fresh target seed candidates (standalone audit retain)

両方とも本 composition では reject だが、 standalone seed として独立 SEED_KERNEL 追加候補:

1. **invention-1 target**: 「proprioception(body)=NEITHER 固有感覚は語れないが確かに存在する」 = W-48 Negative Capability + Sherrington 1906 + Merleau-Ponty 1945 phenomenology anchor candidate
